This is an adapted online version of the brief Mini-Cog cognitive screening test. It may help reveal signs of clinically important cognitive impairment, but it does not replace an in-person evaluation.

Read the instructions carefully and complete the tasks before choosing your score. It is best to take the test with a helper who can read the words aloud and assess the clock drawing more objectively.

Delayed recall of three words
What to do:
Ask a helper to slowly say three words: 'lemon', 'key', and 'ball'. Repeat them immediately, then complete the next task. After a few minutes, say the same three words again without any prompts.

How to score:
Give 1 point for each word recalled without prompting. Choose the total from 0 to 3.

Clock drawing
What to do:
On a blank sheet of paper, draw a round clock face, place the numbers from 1 to 12, and set the hands to show 8:20.

How to score:
Choose 'the clock is drawn correctly' if all numbers are present once, in the correct order, and the hands show the requested time. If there are noticeable mistakes in the numbers, spacing, or hands, choose the error option.
